Whole Person Care

Whole Person Care (WPC) is a Medi-Cal 2020 waiver-funded pilot program. As its name suggests, WPC is designed to help improve the overall quality of life for individuals with chronic medical conditions. This program focuses on individuals with high disease burden, high-risk and high-utilizing individuals. We, along with our local community partners, can deliver seamless care that addresses both the physical, mental, and social needs of an individual.

What services does WPC provide?

We offer services that address physical, mental, and social needs. These services include medical care, mental health services, housing navigation, food stability, health educator, wellness courses, job readiness and training, transportation to and from medical appointments, and even assistance with signing up for and renewing Medi-Cal. Every client has a primary care doctor and a care coordinator.

The care coordinators make our program unique compared to traditional health care. The Care Coordinators speak to clients regularly and assess any needs, which include making appointments, getting medication refills, arranging food basket pick-up, helping with referrals.

Do I have to pay for services?

If you receive Medi-Cal benefits, you may be eligible for the program, at no cost to you.

How do I know if I am eligible for WPC?

There are 3 ways that you apply for the program:

Call one of our care coordinators at 661.326.5231 between the hours of 8:00 a.m. to 5:00 p.m. Monday through Friday. You can also send us an email at WPC@kernmedical.com.

Please include your name, date of birth, phone number and last 4 of your social security number. This is a secure email, and your information will only be used to verify your Medi-Cal eligibility
