For patients with diabetes, we provide individualized support to help manage blood sugar and reduce the risk of complications. Services may include medication adjustments (including insulin and non-insulin therapies), glucose monitoring review, lifestyle counseling, and coordination of care with your primary care team.
For hypertension and hyperlipidemia, our pharmacists focus on reducing your risk of heart attack and stroke. We help optimize blood pressure and cholesterol management, monitor labs, and support healthy lifestyle changes alongside medication therapy.
Our pharmacists play an active role in addiction medicine by supporting medication-assisted treatment (MAT) and recovery. We provide compassionate, stigma-free care, ensure safe and effective use of medications, monitor progress, and collaborate closely with behavioral health and primary care providers.
For individuals living with HIV, our pharmacists help manage antiretroviral therapy, monitor for drug interactions and side effects, and support adherence to maintain viral suppression and long-term health. We work as part of a multidisciplinary team to deliver coordinated, comprehensive care.
Our program is built on collaboration. Clinical pharmacists work side-by-side with physicians, nurse practitioners, physician assistants, nurses, and other healthcare professionals to ensure your care is seamless and well-coordinated.
We are committed to providing accessible, compassionate, and evidence-based care that empowers you to achieve better health outcomes. If you are living with a chronic condition and would like additional support, ask your provider about a referral to our Ambulatory Care Clinical Pharmacy Program.
